Association between obesity and vascular age in patients living with HIV at a national referral hospital in 2023 and 2024Abstract
Introducción: La edad vascular es un indicador emergente que estima el riesgo cardiovascular al reflejar el envejecimiento funcional del sistema vascular. Este estudio tuvo como objetivo evaluar la asociación entre obesidad y edad vascular en personas viviendo con VIH atendidas en el Hospital Nacional Guillermo Almenara Irigoyen entre 2023 y 2024. Además, se analizaron variables clínicas, metabólicas y relacionadas con el VIH para identificar factores asociados a un mayor envejecimiento vascular. Métodos: Se realizó un estudio observacional, analítico, de corte transversal, con enfoque cuantitativo. Se incluyeron pacientes ≥18 años con diagnóstico confirmado de VIH, control regular mínimo de seis meses y datos clínicos completos. La edad vascular se estimó mediante la fórmula de D’Agostino y la obesidad fue definida como IMC ≥30 kg/m². Se seleccionó una muestra de 230 pacientes mediante muestreo no probabilístico consecutivo. Se usaron análisis bivariados y multivariados, aplicando regresión de Poisson con varianza robusta. Resultados: Del total de pacientes, el 16.09% presentaba obesidad. No hubo diferencias significativas en edad ni sexo entre obesos y no obesos. Sin embargo, los pacientes obesos tuvieron menor tiempo de infección por VIH, mayor prevalencia de diabetes y elevación de presión arterial, glucosa y triglicéridos. La edad vascular media fue mayor en obesos (60.9 vs. 52.7 años, p = 0.0052), con mayor proporción en categoría de alto riesgo cardiovascular. Conclusiones: Existe una asociación significativa entre obesidad y mayor edad vascular en paciente con VIH, así como alteraciones metabólicas relevantes. Se requieren estudios longitudinales para confirmar estos hallazgos.Background: Vascular age is an emerging indicator that estimates cardiovascular risk by reflecting the functional aging of the vascular system. The objective of this study was to examine the association between obesity and vascular age in people living with HIV treated at the Guillermo Almenara Irigoyen National Hospital between 2023 and 2024. Additionally, clinical, metabolic, and HIV-related variables were analyzed to identify factors associated with increased vascular aging. Methods: An observational, analytical, cross-sectional study with a quantitative approach was conducted. Patients aged ≥18 years with confirmed HIV diagnosis, regular follow-up for at least six months, and complete clinical data were included. Vascular age was estimated using the D’Agostino formula, and obesity was defined as BMI ≥30 kg/m². A total of 230 patients were selected through non-probabilistic consecutive sampling. Bivariate and multivariate analyses were performed using Poisson regression with robust variance. Results: Among the patients, 16.09% were obese. The analysis revealed no significant variation in chronological age or sex between individuals categorized as obese and those without obesity. However, obese patients had a shorter duration of HIV infection, a higher prevalence of diabetes, and elevated levels of blood pressure, glucose, and triglycerides. Vascular age was significantly higher in obese patients (60.9 vs. 52.7 years, p = 0.0052), along with a greater proportion falling into the high cardiovascular risk category. Conclusions: There is a significant association between obesity and increased vascular age in PLWH, along with notable metabolic alterations. Further longitudinal studies are required to confirm these findings.
