Since 2012-2013 influenza season, World Health Organization (who) recommends the formulation of tetravalent vaccines. Globally, many countries already use tetravalent vaccines in their national immunization programs, while in Latin America only a small number. Two Influenza b lineages co-circulate, their epidemiological behavior is unpredictable. On average they represent 22.6% of influenza cases and more than 50% in predominant seasons. The lack of concordance between recommended and circulating strains was 25 and 32% in the 2010-2017 and 2000-2013 seasons, respectively. There are no clinical differences between influenza A and B. It occurs more frequently from five to 19 years of age. Influenza b has a higher proportion of attributable deaths than influenza a (1.1 vs. 0.4%), or 2.65 (95% ci 1.18-5.94). A greater number of hospitalizations when the strains mismatch (46.3 vs. 28.5%; p <.0001). Different evaluations have demonstrated its cost effectiveness. The compilation of this information supports the use of quadrivalent vaccines in Latin American countries.Type

There are some historical descriptions of this condition by ancient Greek physicians, and the etiological agents have been known only for the last eight decades. The causative agent is the influenza virus, which has three main types: A, B, and C. Type A is capable of circulating within many different biological reservoirs, including humans, swine, and birds. It also has high genetic variability, which allows it to have minor antigenic drifts or mutations which are responsible of epidemics in humans. Sometimes changes are quite marked, leading to pandemics. In a globalized world, with more than 6 billion inhabitants, with many social inequities and evident climate changes, influenza viruses are a permanent risk for mankind. Clinical features for the different viral subtypes may vary from subtle infections to full blown and severe, life-threatening forms. Event with the great advances in diagnostics and immunization, the manufacture and distribution of new diagnostic kits may take some time, and new vaccines are not always readily available. Specific therapies against influenza are not well developed. There are two groups of drugs, the so called adamantane derivatives, such as amantadine and rimantadine, and the neuraminidase inhibitors, such as oseltamivir and zanamivir. Most vulnerable areas for the new flu pandemics include developing countries, particularly the poorest ones, so that the greatest effort must be made for helping these areas.Acceso abierto

Niños hospitalizados con neumonía por influenza AH1N11/2009 pandémico en un hospital de referencia de Perú.Miranda-Choque, Edwin; Ramírez, Carlos; Candela-Herrera, Jorge; Díaz, Javier; Fernández, Ana; Kolevic, Lenka; Segura, Eddy R.; Farfán-Ramos, Sonia (Instituto Nacional de Salud (INS), 2014-03-25)ObjectiveTo determine the clinical and demographic characteristics of pneumonia with influenza virus AH1N1/2009 pandemic at the National Institute of Child. Methods. Retrospective case series in children hospitalized for influenza pneumonia pandemic AH1N1/2009 in a pediatric hospital. Reviewed the medical records between the months of June to September 2009. All cases had virological confirmation, we describe the clinical characteristics and conditions of severity. Results. A total of 74 children of pneumonia with influenza virus AH1N1/2009 pandemic (NVIp), of those 50 were community acquire pneumonia viral (NACv) and 24 pneumonia nosocomial viral (NNv), 16 required mechanical ventilation. 12 died, all had preexisting factors. NN cases showed statistical association with mortality. The most frequent factors were malnutrition, respiratory infections, congenital heart disease and neurological deficits In NACv cases the children under 6 years accounted for 72% (36/50). The median disease duration was 5 days. The most frequent symptoms were fever, cough, runny nose. Received oseltamivir 82%. The chest radiograph 48% of cases showed patchy infiltrates and 44% interstitial infiltrate on chest radiograph. Protein c reactive (CRP) more than 10mg / L was significantly associated with respiratory failure (p <0.05). Conclusions. Cases of NN found who had more mortality, even those who had the highest PCR and those with preexisting condition.Acceso abierto

Características demográficas y geográficas de la mortalidad por la pandemia de influenza en 1918-1920, en tres regiones del PerúMunayco Escate, Cesar Vladimir; Camayo Espinoza, Leonor; Francia Vicente, Rosario de Jesús (Universidad Peruana de Ciencias Aplicadas (UPC)PE, 2022-07-21)Introducción: La pandemia de influenza provocó la muerte de 50 millones de personas en todo el mundo durante su primer año, convirtiéndose en un problema de salud pública y generando así una crisis sanitaria, académica, biopsicosocial y económica. Por ello, estas características la vuelven más interesante de analizar, para así poder enfrentar futuras pandemias de influenza y de otros virus como el SARS-COV 2. Objetivo: Describir las características demográficas y geográficas de la mortalidad durante la pandemia de la influenza 1918-1920 en la región Lima, Loreto e Ica. Materiales y métodos: Estudio de tipo ecológico que contó con el permiso de la CDC-Perú para acceder a la base de datos de registros de fallecidos durante los años de 1915-1922 de las regiones de Lima, Loreto, Ica y La Libertad. Se evaluó mediante el software de lenguaje R: las características demográficas de los fallecidos por causas respiratorias y no respiratorias, el exceso de mortalidad y la identificación de las olas pandémicas en la serie de tiempo de cada una de las tres regiones. Resultados: Se trabajó con 70036 registros de fallecidos, en el cual se identificó que la curva de mortalidad tuvo forma de W, siendo los adultos jóvenes pertenecientes al sexo femenino y los niños menores de 15 años los grupos más afectados. En Lima hubo la presencia de 3 olas pandémicas, siendo la de enero de 1920 la más alta, Ica presentó 2 olas pandémicas que no tuvieron un gran impacto en la mortalidad, en cambio la región de Loreto tuvo una única ola pandémica que tuvo un tiempo de duración de 8 meses. Además, se determinó que en Lima e Ica la principal causa de defunción fue de origen respiratorio, en cambio en Loreto fue de tipo gastrointestinal. Se encontró que en la semana 51 hubo un elevado exceso de mortalidad en Ica y Lima, siendo esta última región la que presentó un mayor exceso global con 1594 (18,1%), Loreto presentó un exceso global de 202 (52,7%). Conclusiones: El análisis identificó que la región más alejada de la capital presentó un mayor índice de mortalidad, que las personas de etnia mixta y que tenían oficios fueron los más afectados. Por ello, debemos crear estrategias de salud pública para las futuras pandemias.Acceso abierto
