dc.description.abstractObjetivo: Valorar la asociación entre el índice neutrófilos/linfocitos (NLR) y sobrevida en pacientes con enfermedad renal crónica (ERC). Métodos: Realizamos un estudio tipo cohorte retrospectiva en los pacientes con ERC de todos los estadíos de la Unidad de Manejo de Enfermedad Renal Crónica del Hospital Edgardo Rebagliati Martins entre Enero del 2016 y Junio del 2018. La variable de exposición fue el índice neutrófilos/linfocitos elevado (NLR> 3.5) y la variable de resultado fue la sobrevida durante el periodo de seguimiento. Se utilizó el método de Kaplan-Meier para la descripción de la función de sobrevida. y regresión de Cox cruda y ajustada para el cálculo del Hazard Ratio (HR) con un IC de 95%. Resultados: Se analizó en total 343 participantes, teniendo una media de edad de 78.3 (±11.9) y una proporción de varones de 62.9%. La prevalencia de NLR incrementado en la población fue de 14,85% y la mortalidad general fue de 17.6%. La mediana del tiempo de seguimiento fue de 2,45 años. La función de sobrevida al final del seguimiento para los pacientes con NLR normal fue de 76.6% (IC95% 0.68-0.83) y para el grupo NLR incrementado fue de 63.5% (IC95% 0.40-0.79). Hubo diferencia significativa entre las curvas de supervivencias de ambos grupos. En el análisis multivariado, la variable NLR elevado obtuvo un HR ajustado de 2,11 (IC95%:1,10-4,06). Conclusiones: En nuestro estudio, el NLR elevado se comportó como un factor de riesgo independiente de mortalidad por todas las causas en pacientes con ERC.en_US
dc.description.abstractObjective: To assess the association between the neutrophil / lymphocyte ratio (NLR) and survival in patients with chronic kidney disease (CKD). Methods: We conducted a retrospective cohort study in patients with CKD of all stages of the Chronic Kidney Disease Management Unit of Edgardo Rebagliati Martins Hospital between January 2016 and June 2018. The exposure variable was an increased neutrophil / lymphocyte ratio (NLR> 3.5) and the outcome variable was survival during the following period. The Kaplan-Meier method was used for the description of the survival function and the crude and adjusted Cox regression for the estimation of the Hazard Ratio (HR) with a 95% CI. Results: A data of 343 patients was analyzed. The mean age was 78.3 (± 11.9) and the male prevalence was 62.9%. The proportion of increased NLR in the population was 14.85% and the overall mortality was 17.6%. The survival function at the end of the follow-up for patients with normal NLR was 76.6% (95% CI 0.68-0.83) and, for the increased NLR group, it was 63.5% (95% CI 0.40-0.79). The median follow-up time was 2.45 years. There was a significant difference between the survival curves of both groups. In the multivariate analysis, the increased NLR variable obtained an adjusted HR of 2.11 (95% CI: 1.10-4.06). Conclusions: In our study, the NLR behaved as an independent risk factor for all-cause mortality in patients with CKD.en_US
