Association between type of health insurance and preventive realization of colonoscopy in ambulatory patients 50 to 75 years old in Peru, 2016Abstract
Introducción: El cáncer colorrectal (CCR) es el cuarto cáncer más común a nivel mundial y en Perú es el segundo cáncer digestivo más frecuente. Su tamizaje mediante colonoscopía partir de los 50 años ha demostrado reducir la mortalidad por CRC. Sin embargo, el acceso al tamizaje no es homogéneo en la población y el seguro de salud podría ser un determinante importante. Objetivos: Evaluar si existe asociación entre el tipo de seguro de salud y la realización de colonoscopía preventiva en adultos peruanos de 50 a 75 años usuarios de consulta externa, en Perú. Métodos: Se realizó un estudio transversal analítico utilizando los datos de la Encuesta Nacional de Satisfacción de Usuarios en Salud (ENSUSALUD) 2016. Se preguntó por la realización de colonoscopía preventiva en los últimos 10 años y el tipo. Se calcularon razones de proporción crudas y ajustadas por potenciales confusores. Resultados: La edad promedio fue 60,5 años (d.e.: 6,8) y 54.8% fueron mujeres, en la población total. La proporción de haberse realizado una colonoscopía preventiva fue 11.8% [IC95%: 8,1% a 15,5%] En el análisis ajustado, los pacientes con seguro privado [RPa: 4,48; IC95%:1,18 a 18,63] y FFAA [RPa: 6,38; IC95%:1,66-24,63] tuvieron mayor proporción de haberse realizado una colonoscopía en comparación a los pacientes sin seguro. No se encontró diferencia significativa entre los seguros SIS y EsSalud. Conclusiones: Los pacientes con seguro privado, seguro de las FF.AA fueron los que en mayor proporción se realizaron una colonoscopía preventiva en comparación a los no asegurados. Mientras tanto, los asegurados al SIS, EsSalud y los no asegurados, no se encontró diferencias estadísticamente significativas.Introduction: Colorectal cancer (CRC) is the fourth most common cancer worldwide, and in Peru, it is the second most frequent digestive cancer. Colonoscopy screening after 50 years of age has shown to reduce CRC mortality. However, access to screening is not homogeneous in the population, and health insurance could be an important determinant. Objectives: To assess whether there is an association between the type of health insurance and the performance of preventive colonoscopy in Peruvian adults aged 50 to 75 years, users of external consultation, in Peru. Methods: An analytical cross-sectional study was carried out using data from the National Survey of Satisfaction of Users in Health (ENSUSALUD) 2016. It was asked about performing preventive colonoscopy in the last ten years and the type. Prevalence ratios calculated and adjusted for potential confounders were calculated. Results: The average age was 60.5 years (s.d.: 6.8) and 54.8% were women. The prevalence of having performed a preventive colonoscopy was 11.8% [95% CI: 8.1% to 15.5%] In the adjusted analysis, patients with private insurance [RPa: 4.48; 95% CI: 1.18 to 18.63] and SSFFAAPP [RPa: 6.38; 95% CI: 1.66-24.63] were more likely to have had a colonoscopy compared to uninsured patients. No significant difference was found between SIS and EsSalud insurance. Conclusions: Patients with private insurance, insurance of the FF.AA. were more likely to have undergone a preventive colonoscopy compared to the uninsured. Meanwhile, the SIS insured, EsSalud and the uninsured, no statistically significant differences were found.
