Admission to the Neonatal Intensive Care Unit in newborn of pregnant women with early and late preeclampsia at Hospital San Bartolomé in 2015 and 2016Abstract
Objetivo: Determinar el riesgo de admisión a la Unidad de Cuidados Intensivos Neonatal (UCIN) de los hijos de gestantes según el tipo de pre-eclampsia por tiempo de inicio. Materiales y Métodos: Estudio cohorte retrospectivo realizado en el Hospital Nacional Docente Madre Niño San Bartolomé durante los años 2015 y 2016 que incluyó a las gestantes que cumplieron con el diagnóstico de pre-eclampsia. Se determinó el riesgo relativo de admisión a UCIN de los hijos de gestantes con pre-eclampsia de inicio temprano (PTe) y pre-eclampsia de inicio tardío (PTa), mediante la regresión de Poisson con varianza robusta y modelo ajustado. Resultados: Este estudio incluyó a 369 gestantes que cumplieron el diagnóstico de preeclampsia. El porcentaje de admisión a UCIN fue 7,0% y la incidencia de preeclampsia durante los años 2015 y 2016 fue 3,04%. En el análisis bivariado, el tipo de pre-eclampsia (PTe o PTa), sepsis y convulsiones neonatales, peso al nacer, peso para la edad gestacional, puntaje de Apgar y mortalidad perinatal fueron estadísticamente significativas (p<0,05) para el ingreso a UCIN. En el análisis multivariable ajustado, los hijos de las gestantes con PTe tuvieron mayor riesgo de ser admitidos a UCIN que los hijos de las gestantes con PTa (RRa= 19,51; IC 95% 9,74-39,11; p<0,001). Conclusiones: Los hijos de las gestantes con PTe tienen mayor riesgo de ser admitidos a UCIN a comparación de los hijos de las gestantes con PTa.Objective: Determine the admission risk of neonates to Neonatal Intensive Care Unit (NICU) from pregnant women with early onset pre-eclampsia (EOP) compared to those from pregnant women with late onset pre-eclampsia (LOP). Materials and Methods: Retrospective cohort study executed at Hospital Nacional Docente Madre Niño San Bartolomé, which included pregnant women who met the diagnosis of pre-eclampsia. The risk ratio of admission to the NICU from pregnant women with early onset pre-eclampsia (EOP) and late onset pre-eclampsia (LOP) was evaluated through Poisson regression with robust variance and adjusted by the confounding variables. Results: This study included 369 pregnant women who met the diagnosis of pre-eclampsia. The percentage of admission to the NICU was 7,0% and the incidence of pre-eclampsia during the years 2015 and 2016 was 3,04%. In the bivariate analysis, the variables type of pre-eclampsia (EOP and LOP), sepsis and neonatal seizures, birth weight, weight for the gestational age, Apgar score and perinatal mortality were statistically significant (p <0.05) for admission to NICU. In the adjusted multivariate analysis, the children of pregnant women with EOP had a higher risk of being admitted to NICU than those of pregnant women with LOP (aRR = 19,51; 95% CI 9,74- 39,11; p<0,001). Conclusions: The neonates of pregnant women with EOP have a higher risk of being admitted to the NICU compared to the neonates of pregnant women with LOP.
