Introducción: el abandono del tratamiento de la tuberculosis (TB) en el Perú es un factor importante para el aumento de los casos de multidrogoresistencia (MDR). Es relevante conocer sus factores de riesgo para prevenirlo y controlarlo. Objetivo: determinar la frecuencia de abandonos de tratamiento de TB y sus factores de riesgo en el Centro Nacional de Excelencia en Tuberculosis Trébol Azul, un centro de atención primaria especializado en TB. Metodología: estudio analítico observacional de cohorte histórica en pacientes que llevaron tratamiento de TB en el centro especializado en Trébol Azul, en San Juan de Miraflores, Lima, 2010-2015. Utilizamos la definición operativa para el outcome abandono según la norma técnica peruana de TB: “Paciente que inicia tratamiento y lo descontinua por 30 días consecutivos o más. Incluye al paciente que toma tratamiento menos de 30 días”. Calculamos las medidas de frecuencia de variables cualitativas, y resumen y dispersión para las variables cuantitativas. Para determinar asociación utilizamos las pruebas de Fisher y Chi cuadrado, así como la de suma de rangos de Wilcoxon, con un nivel de significancia de 0,05. Realizamos dos tablas bivariadas, una para el grupo de pacientes con esquema sensible, y otro para el grupo de pacientes con esquema MDR. Solo realizamos análisis multivariado para el grupo de esquema sensible y empleamos el modelo lineal generalizado tipo Poisson con varianza robusta y calculamos los riesgos relativos (RR) crudos y ajustados (RRa) con sus respectivos intervalos de confianza al 95% (IC95%). Resultados: Se incluyó un total de 245 pacientes con TB. El 18% abandonó el tratamiento. Hubo un abandono del 15.2% en esquema de TB sensible y un 29.8% en esquema MDR. Encontramos una alta frecuencia de tratamiento irregular en un 54,7% en toda la población. Y se encontró que consumir drogas ilícitas aumenta el riesgo de abandono 3,66 veces. (IC95%:1,38-9,69). Conclusión: 1 de cada 5 pacientes presenta abandono al tratamiento anti TB. Más de la mitad de pacientes presentaron irregularidad al tratamiento. Se evidenció que consumir drogas ilícitas es un factor de riesgo asociado al abandono del tratamiento anti TB. Background: Tuberculosis (TB) treatment abandonment in Peru is an important factor in this epidemy that persists with a rise on multidrug-resistant (MDR) numbers. Thus, is relevant to know its associated factors in order to prevent and control it. Aim: To determine the frequency of abandonment of TBC treatment and its associated factors in the CENEX Trébol Azul, a primary care center, specialized un TB. Methods: Observational analytical historical cohort study in patients that took TBC treatment in the specialized center CENEX Trébol Azul, in San Juan de Miraflores, Lima, 2005 to 2015. We used the otcome definition “abandonement” by the official Peruvian document about TB: “Patient that’s starts treatment and discontinues it for 30 consecutive days or more. It includes the patient thas take treatment in less than 30 days.” Measures of frequency were calculated for the qualitative variables and resume and dispersion for the quantitative variables. We used Fisher, Chi square tests, and Wilcoxon rank sum, with a significance level of 0.05. We maked 2 bivariate charts, one for the patients with the sensible TB treatment and one for the MDR treatment. We just maked the multivariate analysis for the group with the sensible tratment, and used a Generalized Lineal Model (GLM) Poisson type with robust variance, was used to calculate the Relative Risk (RR) crude and adjusted (RRa) with a confidence interval 95% (95%CI). Results: A total of 245 patients with TB was included. 18% of abandonment of treatment. There were a 15.2% of patients that abandon the sensible treatment and a 29.8% the MDR treatment. We also found a high frecuency oh irregular treatment in a 54,7% en the whole population. It has been found that the consume of drugs increase the risk of abandonment in 3,66 times (IC95%:1,38-9,69). Conclusion: 1 of each 5 patients presents abandonment of TB treatment. More than the half of patients presents irregularity to the treatment. It was concluded that the consumption of illegal drugs is a risk factor associated to the abandonment of TB treatment.Type
