Relationship between the depth of the burn and the presence of nosocomial infections in hospitalized patients of the Burn and Plastic Surgery unit of the Hospital Nacional Arzobispo Loayza, period 2006-2016Abstract
Introducción: La infección nosocomial constituye la primera causa de morbi-mortalidad en el paciente quemado, es una complicación que genera secuelas físicas, psicológicas e incluso muerte. Existen factores de riesgo asociados al desarrollo de estas infecciones como profundidad de la quemadura y la superficie corporal total quemada. Nuestro objetivo es determinar la relación entre profundidad de la quemadura y el riesgo de infecciones nosocomiales en el servicio de Quemados y Cirugía Plástica del Hospital Nacional Arzobispo Loayza (HNAL) durante 2006-2016. Metodología: Se realizó un estudio observacional analítico tipo cohorte retrospectiva en pacientes quemados hospitalizados en un establcimiento especializado. Se obtuvieron riesgos relativos crudos y ajustados (RRa) usando Regresión de Poisson. Resultados: Se incluyeron 131 pacientes en el estudio. La incidencia de infecciones nosocomiales fue de 15.27%. Hubo mayor riesgo de infección nosocomial en pacientes con quemadura grado III y presencia de infección nosocomial en este grupo (RR 4.54; IC95% 1.96-10.49; p<0.001) ajustado por edad, leucocitos, albúmina, uso de sonda Foley, uso de catéter venoso central, localización de la quemadura, superficie corporal total quemada. Hubo menor riesgo de infección por cada gr/dl adicional de albumina (RR 0.258; IC95% 0.12-0.57; p=0.001) Conclusiones: El desarrollo de infecciones nosocomiales se asoció a quemaduras más profundas (Grado III) independientemente de la superficie corporal total afectada; mientras que niveles más altos de albumina se asociaron a menor riesgo. Estos factores deben ser considerados en la prevención de infecciones en pacientes quemados al ingreso.Background: Nosocomial infections are the first cause of morbidity and mortality in burned patients. It is a complication that generates physical, psychological and even death outcomes. There are risk factors associated with the development of these infections, being depth of the burn and burned total body surface among the most important. The aim of this study is to evaluate association between depth of the burn and development of nosocomial infections in patients from the Burns and Plastic Surgery Unit of the “Hospital Nacional Arzobispo Loayza” (HNAL) during 2006-2016. Methods: An observational study, retrospective cohort type was performed on burned patients hospitalized in a tertiary level center. Crude and adjusted Relative Risk (aRR) were obtained using Poisson regression. Results: Of 131 patients included in the study, the incidence of nosocomial infection was 15.27%. There was higher risk of nosocomial infections among patients with Grade III burns (aRR 4.54; 95%CI 1.96-10.49; p<0.001) adjusted per age, WBC, albumin levels, Foley catheter, central venous catheter, burn localization, and total burned body surface. There was less risk for each additional gr/dl of albumin (aRR 0.26; 95%CI 0.12-0.57; p=0.001). Conclusions: Nosocomial infections were associated to deeper burns (Grade III) independent from total burned body surface; meanwhile higher albumin levels were associated to lower risk. These factors should be taken into account to prevent the occurrence of nosocomial infections and its complications among burn patients.
