Objetivo: Identificar la frecuencia y los factores asociados a la falla del tratamiento para la erradicación de Helicobacter pylori (Hp). Métodos: realizamos un estudio de cohorte retrospectiva en el cual se incluyeron pacientes atendidos de forma ambulatoria con diagnóstico de infección por Hp que hayan recibido tratamiento y hayan tenido un control dentro de los tres meses post-tratamiento en una clínica privada entre los meses de enero del 2014 y diciembre del 2015. Se consideró falla al tratamiento cuando presentaba un test de ureasa de aliento positivo entre 1 y 3 meses posteriores a terminado el tratamiento. Se realizó un análisis descriptivo; asimismo, se realizó un análisis bivariado mediante las pruebas de chi-cuadrado o Fisher para las variables cualitativas y U de Mann Whitney para las variables cuantitativas. Para el análisis multivariado se empleó un Modelo Lineal Generalizado de la familia Poisson. Para el análisis de datos se usó STATA v11.0. Resultados: Se incluyó 581 pacientes, de los cuales 58,5% fueron mujeres. La edad promedio fue de 48,1±14,3 años. El 29,8% presentaron falla al tratamiento. La terapia triple estándar tuvo una frecuencia de falla de 33,3%. Los pacientes que usaron terapia cuádruple con salicilato de bismuto como terapia tuvieron 55% menos riesgo de presentar falla al tratamiento en comparación con las demás terapias usadas. Asimismo, quienes habían presentado una infección previa por Hp tuvieron 1,73 veces más riesgo de tener falla al tratamiento frente a los que no presentaron dichas condiciones. Conclusión: se encontró una alta frecuencia de falla al tratamiento contra Hp utilizando la terapia triple estándar. Los factores asociados hallados fueron infección previa y uso de terapia cuádruple. METHODS: We conducted a retrospective cohort study in which patients were included if they were diagnosed with Hp infection, have been treated and have returned for follow-up within three months post treatment in a private clinic between the months of January 2014 and December 2015. It is considered treatment failure when urease breath test is positive between 1 and 3 months after treatment ended. A descriptive analysis was performed; also, a bivariate analysis using the chi-square test or Fisher for qualitative variables and U Mann Whitney for quantitative variables. For the multivariate analysis, a Generalized Linear Model of Poisson was used. For data analysis, we used STATA v11.0. RESULTS: We included 581 patients, of whom 58.5% were women. The average age was 48,1±14,3 years. The treatment failure was 29.8%. The standard triple therapy had a failure rate of 33.3%. Patients who had had a previous infection had 1.73 times higher risk of treatment failure compared to those who did not present this condition. Likewise, those who had use of quadruple therapy with bismuth salts in the treatment had 55% less risk of treatment failure in comparison with the other therapies used. CONCLUSION: A high frequency of failure was found to Hp treatment using standard triple therapy. The associated factors that were find were previous infection and use of quadruple therapy with bismuth salts.
